Your child · Family Evidence Map
Set up your family workspace
Track patterns. Preserve context. Prepare better conversations with school or clinicians.
One family, one child profile to begin with. Nicknames are fine, and you can change any of this later.
Boundaries
Before you start
Please read this once.
This app does not diagnose ADHD, autism, anxiety, depression, trauma or any medical condition. It does not rule anything in or out, score or rank your child, assess risk, interpret imaging or infer biomarkers. It helps organise observations and questions for a qualified professional.
What this is
- A family-written record of what you notice, day by day
- Context: settings, timing, what came before, what helped
- Signals to explore with a qualified professional
- Questions to take into a school or clinical conversation
What this is not
- Not a diagnosis of ADHD, autism, anxiety, depression or trauma
- Not a screener: it cannot rule a condition in or out
- Not a score, rating, ranking or risk assessment of your child
- Not imaging, biomarkers or any medical interpretation or treatment advice
You are collecting structured evidence about attention, regulation, sleep-linked functioning, sensory load, transitions, recovery, strengths, supports and impact across settings — so you can decide, calmly, whether to seek a professional assessment. If you are worried about safety, regression, severe distress, or daily functioning, seek professional help now.
Audience
Who this is for
Families first.
- You and your child — the primary users. You keep the record and decide what happens with it.
- A teacher, tutor, coach or relative — optional. They can add a short weekly note about their own setting, nothing more.
- A GP, therapist, SENCO or OT — a recipient of the Assessment Prep Report if you choose to share it. They do not use the app.
